Extra Help Medicare: Who Qualifies and How to Apply

What is Extra Help with Medicare?

In short: Extra Help Medicare is a program that pays toward Part D drug plan costs when income and savings are low. In 2026 the limits for one person are $23,940 in income and $18,090 in resources.

A pharmacy counter is often where families first hear of it. The total is more than your mother expected, and nobody had mentioned there was help. That's the part nobody explains. If the paperwork is what piles up once you know, a navigator can make those calls and track them. Navigate Care explains how that works and what it may cost on its what you will pay page.

Now the detail. Medicare.gov describes the program as help for people with limited income and resources to pay Medicare drug coverage (Part D) costs. It is for prescriptions. It doesn't pay hospital or doctor bills. Our Medicare Part D guide explains the plans themselves.

What Extra Help changes in your Part D costs

According to Healthline, Extra Help can cover Part D premiums, deductibles and copayments. In place of the full price, your parent pays a small set amount at the counter.

Medicare.gov has published what you'll pay under Extra Help in 2027, and those are the figures to plan around for open enrollment:

What your parent pays per drug under Extra Help in 2027

Drug typeMost your parent pays per drug
Generic drugUp to $5.80
Brand-name drugUp to $14.40
Any covered drug, with full Medicaid and QMBNo more than $4.90

I haven't found the 2026 copay amounts on a page I could read, so I'm not quoting them. Your parent's plan, or the number on the back of the card, will say what applies today.

Who qualifies for extra help Medicare in 2026

Medicare.gov says that in most cases you must have income and resources below a certain limit. Resources means money and things you could turn into money: bank accounts, for example. Both tests apply.

One person$23,940$18,090

These are the "in most cases" numbers, so a case that sits near the line is worth a call to SHIP. A form takes little time. We can't tell you how an application will turn out.

Some people don't need to apply at all. Medicare.gov says you get Extra Help automatically if you have certain other help: full Medicaid, a Medicare Savings Program, or SSI. If your parent has one of those, check their drug plan before filling in anything.

How to apply

Do these in order.

  1. Gather what you know: income for the year and the balances of bank accounts. Healthline says you don't need to send income or asset documents with the form, and you don't list a home or car.
  2. Apply through Social Security. Healthline says you can use the SSA website or mail in the form, and a mailed form has to be an original, not a photocopy. Social Security's own page returned an error when I tried to read it, so start from Medicare.gov, which links to it.
  3. Ask for free help if you want it. Medicare.gov says your local State Health Insurance Assistance Program, SHIP, gives free help applying. You can also call Medicare on 800-633-4227, according to Healthline.
  4. Check at the pharmacy. Once your parent is approved, make sure the lower copays have actually started.

Timing is worth a thought. Open Enrollment runs October 15 to December 7, 2026, the yearly window to change a Part D plan. If your parent is likely to qualify, it helps to know the answer before choosing a plan. SHIP can look at both questions in one call.

Extra Help or a Medicare Savings Program?

They are different, and they sit next to each other. Extra Help is about drug costs. A Medicare Savings Program helps pay Medicare premiums, like the standard Part B premium of $202.90 a month in 2026.

The 2026 monthly income limits for the Savings Programs are $1,350 for one person under QMB, $1,616 under SLMB and $1,816 under QI, with $1,824, $2,184 and $2,455 for a married couple. The resource limits are $9,950 and $14,910. Those are lower than Extra Help's, which is why a parent can fail one test and pass the other. Getting a Savings Program is also one of the ways Extra Help arrives on its own.

If this is a tax or money-planning question, take it to SHIP, Medicare or Social Security. We can explain how the program works. We can't tell you what to do with your finances.

Where a navigator comes in

A navigator doesn't treat anyone or give medical advice, and doesn't replace SHIP. What a navigator can do is the follow-through: tracking the application, checking the pharmacy applied the new copay, calling the plan when it didn't. Medicare doesn't automatically cover navigation. Eligibility depends on a qualifying condition or need and a provider visit, and we confirm your cost before you start. It is usually $0 with supplemental coverage.

Frequently asked questions

Is Extra Help the same as Medicaid?

No. Extra Help pays toward Part D drug costs. Full Medicaid is a separate program run by your state, though having it means you get Extra Help automatically, according to Medicare.gov. Our Medicaid vs Medicare guide explains the split.

Does Extra Help cover Medicare Part B premiums?

No. That is what a Medicare Savings Program is for. Extra Help is about drug costs.

Do I have to apply for Extra Help every year?

I couldn't find a page that answers that, so I won't guess. Ask SHIP or call 800-633-4227, and keep any letter the program sends you.

What will my parent pay for a drug with Extra Help?

In 2027, up to $5.80 for a generic and up to $14.40 for a brand-name drug, according to Medicare.gov. With full Medicaid and QMB it is no more than $4.90 for each covered drug.

Where do I apply for Extra Help?

Through Social Security, online or by mail, according to Healthline. SHIP will help you with it for free.
