Contracted Position
Conduct brief, structured telehealth visits for Medicare patients who may benefit from ongoing care navigation — establish medical necessity, document what matters, and set a care plan the navigation team can act on.
About Navigate Care
Navigate Care helps older adults manage the medical and social challenges affecting their health. Our care navigators coordinate medical appointments, transportation, medical equipment, community resources, caregiver support and the other services patients need to remain healthy and independent.
About the role
As a Virtual Nurse Practitioner at Navigate Care, you will conduct brief, structured telehealth visits for Medicare patients who may benefit from ongoing care-navigation services.
You will identify serious medical conditions and unmet social needs, establish medical necessity, and develop individualized care plans for ongoing support.
What you’ll do
- Conduct initiating telehealth visits
- Review patients’ medical conditions, medications, functional limitations and social needs
- Determine medical necessity for care-navigation services
- Document diagnoses, risk factors and applicable SDOH Z-codes
- Develop and approve individualized care plans
- Collaborate with physicians, care navigators and the clinical operations team
- Review clinical concerns escalated by care navigators
- Complete accurate and timely documentation in the Navigate Care platform
What you bring
- Completion of an accredited Nurse Practitioner program
- Active and unrestricted Texas NP or APRN license
- Current national board certification
- Individual NPI and active Medicare enrollment
- Two or more years of experience in primary care, geriatrics, telehealth or care management
- Experience working with Medicare patients
- Familiarity with care navigation and SDOH documentation
- Comfort using EHRs, telehealth systems and care-management platforms
- Strong communication and clinical-documentation skills
Bilingual English and Spanish proficiency is strongly preferred.

The arrangement
This is a remote, part-time, contracted 1099 position. Visit volume varies with patient referrals and program needs. There is no guaranteed minimum, and you are free to work with other organizations.
Compensation is $75 to $85 per clinical hour, depending on experience, qualifications, languages and availability. Visits are structured and supported by an experienced care-navigation team, so clinical time is spent on clinical work.
An active, unrestricted Texas NP or APRN license, current national board certification, an individual NPI and active Medicare enrollment are required before the first visit.
